Card and Driver’s License.
2. Residence addresses for the past 2 years.
3. If renting, please provide landlord name, phone
number and addresses for the past 2 years.
4. Employer’s names, phone numbers and addresses
for the past 2 years.
5. W2’s and or 1099s for all employers for the
past 2 years. Current pay stubs from the past 30 days.
6. Just out of college, include copy college transcripts
and copy of Diploma.
7. Retired, provide most recent awards letter (SSI)
or pension statement.
8. Current month and past 2 months bank statements
for all asset accounts (checking, savings, 401k, etc.)
Please send all pages, even if blank.
9. Addresses & mortgage information for any real
estate you own. If any of these properties are rentals,
please provide lease agreements.
10. If self-employed, include last 2 years of personal
and corporate Federal Tax Returns and Year-to-Date
Profit and Loss.
11. If purchasing a home, the fully executed Purchase
Contract.
12. Certificate of Eligibility and DD214, if applicable.
13. Bankruptcy papers, if applicable.
14. Divorce decree, if applicable.
15. Proof of child support and or spousal maintenance,
if applicable.
16. Checks payable to: IndyMac Bank
